The Order Information portion includes the Lab name, Client/Facility, Ordering Physician and Client ID.
The following table describes the Order Information fields:

Lab |
This field is populated by the system and it defaults to the lab associated with the facility selected. |
Client/Facility |
The Client/Facility may be either the practice name or a physician's name. |
Ordering Physician |
The Ordering Physician is the physician ordering the lab work. This field defaults to the Client/Facility. Users may choose another physician from the drop-down list or search for one using the search icon. For more information see To Find a Caregiver in the Common Procedures section of this guide. |
Client ID |
The Client ID is the laboratory's account number for the office or physician. The system populates this field based on your Client/Facility selection. A Client ID is a unique code assigned by a sponsor, to identify a provider (Physician or Facility). Every Client/Facility has a default Client ID. Physicians who do not have a Client ID assigned to them by the sponsor are assigned the default Client ID of the Facility that they are associated with. |
Date/Time |
This is the date and time when the order is created. |
Order Number |
This field is used by some labs. It allows users to enter an order number. |
STAT |
This field is used by some labs. Checking this field indicates that the ordering physician wants STAT processing of this order. The definition of STAT is determined by the performing lab. |
Prepaid Amount |
This field is used by some labs. It is used to enter any amount pre-paid by the patient for the order. This amount is printed on the order. |
