Rx Pad

The Rx Pad electronic form is used to generate new prescriptions. This screen displays the physicians, ICD-9 codes, drugs, SIG codes and pharmacies associated with each location. Users can search for and select other drugs or other pharmacies by selecting the Search icon (binoculars) next to each field.

If the patient has a PBM (Prescription Benefit Plan), after a drug is selected a list of alternate drugs displays. The user has the option of selecting an alternate drug or continue with the current drug selection. After a drug is selected, co-pay information for the drug appears at the bottom of the Rx Pad screen.

The Alt button next to the Drug field becomes enabled (by changing to yellow), to allow the user to return to the Drug Alternatives screen and select an alternate drug. If a user does not select a benefit plan, the Alt button stays off.

The following table describes the contents of the Rx Pad screen:

Field/Button

Description

Prescriber

This drop-down contains the list of physicians the user is allowed to prescribe on behalf of.

Supervisor

If the prescriber is required to have a supervising physician when prescribing, this is where it would be indicated.  The name of the supervising physician will also print on a printed script.  For example, in some states, a Physician's Assistant requires a supervising physician when prescribing.

Diagnosis

This is the diagnosis for which the drug is issued. This list contains physician and location ICD9 preferences.  The physician level preferences appear on top and the practice or facility level preferences appear below the line.

To enter a diagnosis:

  • The user can select a preferred diagnosis from the drop-down list or

  • type a diagnosis code and press the Enter key or

  • click the search icon to search for a diagnosis code using the ICD-9 Code Search screen.

Drug

This is the name of the drug being prescribed. This field is populated with physician and location prescription favorites.

If no Diagnosis is selected:

The drug list contains both the physician’s and facility's preferred drugs that are not associated to any diagnosis. Otherwise, the list is empty.

If a Diagnosis is selected:

The drug list contains both the physician’s and facility's preferred drugs associated to the selected diagnosis. Otherwise, the list is empty.

To enter a drug name:

  • Select a preferred drug from the drop-down list or

  • type a drug name and press the Enter key or

  • click the search icon to search for a drug using the Drug Search screen.

Based on the provider and location Rx  preferences, selecting a drug may populate the SIG  field below.

Note: If the Drug you select is a controlled substance, you cannot issue an electronic Rx. (The 'Electronic' option is removed from the Issue Via drop-down).

If benefit/formulary is involved, the following formulary statuses (symbols) will precede the drug name:

  • P = Preferred

  • F = On-Formulary

  • N = Non-Formulary

  • X = Non-reimbursable

  • U = Unknown

Medicaid/InfoScan has 2 additional statuses:

  • ? = Prior Authorization

  • $ = Higher Co-Pay

View Monograph

 

Picking View Monograph provides more detailed information about the drug provided by the manufacturer.  The information is provided by the drug manufacturer.  Examples would include drug interactions, side effects, etc.  (For instance: Avoid eating grapefruit or drinking grapefruit juice while being treated with this medication unless your doctor instructs you otherwise. Grapefruit juice can increase the amount of certain medications in your bloodstream. Consult your doctor or pharmacist for more details.)  These instructions can be printed for the patient.

Alt.

When enabled, clicking the Alt button invokes the Drug Alternatives screen where the user can select a similar drug.  This button is enabled if the patient has an Rx Benefit plan.

  • If a prescription plan was selected, the Alt button becomes enabled when a drug is selected. Clicking the Alt button displays a list of alternative drugs and allows users to either select a different one from the list or ignore the alternatives and keep the same drug.

  • If no prescription plan was selected, the Alt button stays off.

  • The drugs are listed and grouped by formulary status and ordered by most preferred to least preferred.

Transmitted Drug Name

This is the name of the drug that appears on the electronic prescription that is sent to the pharmacy. This field is populated by the system.

Formulary Status

Next to the transmitted drug name is the formulary status with the note 'See Copay and Coverage below'.  The details are displayed at the bottom of the screen under Copay Information and Coverage Information.

The formulary status will display for the patient's benefits referencing the copay and coverage information at the bottom of the screen.  Under the Issue Via field, the Copay and Coverage details will display if provided.

SIG

SIG codes are the abbreviated instructions that are printed on a prescription label, instructing the patient how to take the drug.

If a preferred drug is selected, the SIG drop-down contains all the SIG codes associated with that drug.

The SIG drop-down also gives users the option of selecting  "Show Industry SIGs" or "Show SIG Builder". Once a user makes a selection the other option goes away unless a different drug is selected.

By default the SIG builder is hidden. Selecting the option "Show SIG Builder" displays the SIG Builder.  

  • Multiple SIGs can be associated to a single drug.

  • Duplicate SIGs are not allowed.

  • Days Supply, Quantity, Comments, Refills and DAW are driven by the selected SIG.

    1. If any of the above fields is changed, and the “Save Rx as Preference” box is checked, the updated information will be saved to the SIG in context.

    2. If any of the above fields is changed, and the “Save Rx as Preference” box is NOT checked, the updated information will not be saved to the SIG in context.

View Dosage

Picking View Dosage allows the user to verify the dosage for a patient based on their age, weight and the strength of the drug.  View Dosage is usually used when NOT using an existing SIG as industry SIGs already take the patient's age, weight and the strength of the drug into consideration.  If  View Dosage is selected when the weight field is empty, a message will be displayed stating that weight must be filled in.

Quantity

These are the number of units to be dispensed.

Units of Measure

These are the units of measure for the drug quantity. (Ex: Bottle, Dispenser, Pack etc.)

Weight

When the patient's weight is entered, the data is added to the patient's chart and will automatically populate in the future.  It is only required in dosage checking.  The field is ignored otherwise.

Days Supply

This is the number of days of available drug supply.

Refills

This number indicates the number of times a pharmacy may dispense the prescription after the initial prescription is issued.

DAW

If the DAW box is checked, this instructs the pharmacist to "Dispense the drug as Written".

When the DAW box on a prescription form is checked, this indicates that the prescribing physician has instructed the pharmacy to use the branded version of a drug.  

If the DAW box is unchecked, that means generic versions of the drug are allowed.

Comments

This field contains additional comments from the provider.

Issue To

This is the pharmacy for which the prescription is issued. This field is populated with physician and location pharmacy favorites.

  • The user can select a pharmacy from the drop-down list or

  • type a pharmacy name and press the Enter key or

  • click the search icon to search for a pharmacy using the Pharmacy Search screen.

Issue Via

This is the method used to issue the drug (Electronic, Print, Telephone etc.)

Note: If the Drug you select is a controlled substance, you cannot issue an electronic Rx. (The 'Electronic' option is removed from the drop-down).

Note: 'Reported' drugs are not authorized.  If you issue a prescription as Reported, the newly created prescription will not appear on the Rx Authorizations list.

Do not save as preference

If this button is selected, the Rx is not saved as a preference.

Save as prescriber preference

If this button is selected, the Rx is saved as a prescriber preference. (Saved preferences display under the Admin>Preference Lists>Prescription sub-menu).

Save as facility  preference

If this button is selected, the Rx is saved as a facility preference. (Saved preferences display under the Admin>Preference Lists>Prescription sub-menu).

Rx History

This button takes the user to the patient's Rx History tab on the Patient Chart.  If selected, a Back to Rx Pad button is provided to return the user to Rx Pad.

Issue

This button submits the Rx.  See Issuance and DUR Check for DUR and Issue interaction.

Hold

This button saves all the prescription information without submitting the prescription.

DUR Warnings

DUR - Drug Utilization Review

This feature is designed to alert the user of potential adverse reactions caused by any of the following conditions:

Therapeutic duplication, drug interactions, incorrect dosages, ingredient duplication, or contraindications for gender, age, pregnancy, or disease.

When a user generates a drug prescription, Clinician will display DUR Warnings if any of these conditions is present. The user can either bypass the warning(s) and proceed with issuing the prescription or modify/cancel the prescription.

The number of DUR warnings appear inside the circles.

Note: Failure to review and acknowledge DUR warning(s) will prevent the user from issuing a prescription (based on the DUR preferences for the physician or facility).
